Forms

Medical Leave Form: to be filled up by students in case of loss of attendance due to medical reasons. Student can download the form and submit to SSO, duly signed by student, parent/guardian, and the program coordinator.

General Leave Form: to be filled up by students in case of loss of attendance due to any reason other than medical. Student can download the form and submit to SSO. If necessary, the form can be submitted by parents/ guardians/ classmates.

Individual Indemnity Form: to be filled up by each student individually before going for field trip/ study travel. The form should be submitted to the SSO.

Group Indemnity Form: to be filled up by a group of students going together for group trips like Summer school/ Winter school/ Internship/ Exchange programs. The form should be submitted to the SSO.

Clearance Certificate: to be submitted by graduating students in order to clear all accounts at CEPT U. Student can download the form and submit it to Accounts/ Faculty Admin, duly signed by various offices. A self-attested photocopy of the completed form should be submitted to SSO.
